This voucher entitles the bearer to a 15% discount off followup exams conducted under an Insurance or Vision Plan which limits the Patient to one exam visit per year, or where the Insurance or Vision Care Plan has already been fully utilized for the year.

The Voucher must be presented at time of service and is valid for single use only. The Voucher is transferable and can be used by family and friends of original holder. The Voucher cannot be used in conjunction with any other discount on the same item. The Voucher is designed to be used when there is no coverage by the Patient's insurance or Vision Plan & therefore supplements the Patient's insurance or Vision Plan coverage.

The Voucher may be accepted by, and utilized with other participating practitioners within the EyeVisionNet system.
